How to complete an AVA Authorization Letter for a Singapore Visa
✅ Step-by-Step Instructions to Complete the Authorization Letter
1. Print a Fresh Copy
- Print out a new copy of the Authorization Letter.
- Complete it using wet ink (i.e., pen, no digital or typed entries).
2. Fill in the Blue Highlighted Fields
These fields must be completed by the applicant:
- Applicant’s Full Name
- Passport Type & Passport Number / Identity Card Number
- Full Name of the Authorized Representative (e.g., Peninsula Visa staff member)
- Signature of Applicant
- Date of Signature by Applicant
🖊️ Use a black or blue pen only.
3. Leave Red Highlighted Fields Blank
These fields are for office use (e.g., by Peninsula Visa or the Authorised Visa Agent [AVA]):
- Relationship Declaration (e.g., who is my “agent”)
- Representative’s Passport/ID Details and Contact
- To: and At: (Name and Address/Location of AVA)
- Signature of AVA and Date
🚫 Do NOT fill in any red-highlighted fields.
4. Tick the Green Box for Authorization
This box must be ticked to authorize Peninsula Visa to act on the applicant’s behalf:
- ✅ “Submit my visa application and all necessary documents on my behalf”
🔲 You may also tick the second checkbox if you authorize the representative to:
- “Collect my passport with its visa on my behalf”
5. Signature Section
- At the bottom, the applicant must sign and date the form where indicated (blue highlight).
- Do not fill in the “Accepted by AVA” or “Signature of AVA” sections — these are to be completed by the visa agency.
📌 Important Reminders:
- The entire form must be handwritten — no typed fields allowed.
- Make sure all handwriting is clear and legible.
- Ensure no fields in red are completed by the applicant.
- If any mistakes are made, start over with a fresh printout.
