Who needs to appear at the Acceptance Office Appt for a Minor?

For minors under age 16, both parents or legal guardians must appear in person with the child at the Acceptance Facility. If one parent or guardian is unable to attend, the absent parent must provide a notarized Form DS-3053 (Statement of Consent) along with a photocopy of the front and back of their ID. If only one parent has sole legal custody, supporting legal documentation (e.g., a court order or birth certificate listing only one parent) must be provided.


For minors aged 16–17, the applicant is generally expected to appear in person and can apply on their own. However, it is recommended that at least one parent or legal guardian be present to show parental awareness. In some cases, a signed statement of consent or a parent’s ID may be required.
