Can I request an alias or “also known as” (AKA) name to be printed in my new U.S. passport?

Yes, it is possible to request that an “also known as” (AKA) name be included in your new passport. To do so, follow these steps when completing your DS-82 application: 

Instructions:

  1. Enter the AKA name in Block 9

    In section 9 of the DS-82 application (“List all other names you have used”), write the name you are requesting to be included as your known-as or alternate name.

  2. Sign both names

    You must sign the application using both your legal name and your AKA name.

  3. Submit a written request

    In addition to the DS-82, please include a signed statement requesting that your AKA name be printed in the passport. This statement should include:

    • Your full legal name
    • The AKA name you wish to include
    • A brief explanation for the request
    • Your signature and the date


Example AKA Request Statement:


“I, [Your Full Legal Name], respectfully request that my passport include the name I am also known by: [Your AKA Name]. This is a name I have used consistently and wish to have printed for identification purposes.

Signed,

[Your Full Legal Name]

[Your AKA Name]

[Date]”
